Now accepting resumes for Assistant Program and Case Manager.
1.Creation of Treatment Plans and Discharge Summaries in a timely manner as required by Vermont State Regulations.
2.Monitor resident’s health and facilitate visits with healthcare providers. Assist in providing resident transport as necessary.
3.Assist counselors as needed in order to maintain rules and structure of the program.
4.Understand and maintain compliance with all Vermont State Licensing Regulations for Residential Treatment Programs and Private Non- Medical Institutions (PNMI).
5.Assume duties of the Program Manager in their absence.
6.Communicate with state agencies and families regarding referrals, discharges and transitions.
7.Coordinate and communicate with community service providers to promote growth and change in resident’s lives.
8.Maintain reports and statistics of admissions and discharges.
9.Must be flexible enough to take on extra duties when necessary including on call.
10.Provide shift coverage if needed.
Bachelor’s Degree and experience with at risk youth. 2 years of college and 2 years of experience working with youth may be substituted for above. Master’s degree in a relevant field and one-year experience providing direct care in residential treatment programs for children/youth. Or, Bachelor’s degree and two years of experience providing direct care in residential treatment programs for children/youth. Or,High School Diploma or GED and four years of experience working with children/youth in residential treatment programs.
